We help you take what feels like a confusing process and turn it into clear next steps.That can look like:
Explaining what’s happening and what comes next (in plain language)
Helping you keep track of appointments, referrals, and deadlines
Following up so things don’t get stuck or forgotten
Helping you connect with the right providers and programs
Supporting paperwork and documentation when it’s needed
Problem-solving barriers (transportation confusion, missing information, trouble reaching someone, etc.)
These supports align with Community Supports coordination and ECM-aligned non-clinical care management, based on health plan direction.


Community Supports are services your health plan may approve to help with needs like housing stability and nutrition. If you’re approved, we help you complete steps, stay engaged, and keep services moving.We can support coordination for:
Housing Transition & Sustaining Services (HTSS)
Housing Tenancy Navigation Services (HTNS)
Housing Deposits
Medically Tailored Meals
ECM is for members who need extra support because multiple things are happening at once. If you’re enrolled in ECM, we help you stay on track through outreach, coordination, task tracking, and follow-through.We support:
Outreach/engagement support
Coordination with providers and community partners
Task tracking and follow-up
Documentation support (when needed for services)
Non-clinical support only. Eligibility/authorization is determined by the health plan.

Support may include:
Check-ins and outreach (calls/reminders)
Help organizing next steps and paperwork
Coordination with service providers and community partners
Follow-ups on referrals and service start dates
Identifying barriers and helping get them resolved
Documentation support and progress updates (when needed for services)

My name is Michael Keeton, and I’m the founder of Guided Pathways Community Solutions. I started this organization because I’ve seen too many people lose momentum in the system—not because they didn’t qualify for help, but because the process is hard to navigate.Small things like missed calls, confusing steps, paperwork delays, and lack of follow-up can hold up services for weeks or months. This figure represents the estimated number of individuals experiencing homelessness across San Diego, Riverside, Inland Empire, and Imperial Counties, based on the most recent Point-in-Time counts.
Homelessness trends vary by county, with some regions seeing modest declines while others continue to experience year-over-year increases, underscoring the need for coordinated, housing-focused supports.
Consistent outreach, coordination, and follow-through can reduce delays and help members stay engaged with approved supports.
